The Vaccination Schedule in France: Key Stages

In France, the vaccination schedule is established annually by the Ministry of Health, on the recommendation of the Haute Autorité de Santé (HAS). It specifies the recommended vaccines, the ages at which they should be administered, and the schemes to be followed.

For children aged 0 to 3 years, vaccinations are mainly concentrated during the first months of life, a period when the immune system is still developing and when the protection conferred by maternal antibodies begins to decrease.

The First Weeks: From Birth

The tuberculosis vaccine (BCG) is recommended from birth for infants considered to be at risk, according to criteria defined by the HAS. It is not mandatory for the entire population but strongly advised in certain family or geographical contexts.

At 2 Months: A Key Stage

Around the second month, the vaccination schedule intensifies. Hexavalent vaccines — which protect against six diseases in a single injection (diphtheria, tetanus, polio, whooping cough, Haemophilus influenzae type b, hepatitis B) — are administered according to a multi-dose schedule. The meningococcal serogroup C vaccine and the pneumococcal vaccine (13-valent) complete this program.

Between 4 and 12 Months: Following the Schedule

The following months allow for the completion of the initiated schedules, with essential boosters to ensure lasting immunity. At 11 months, in particular, a booster dose for hexavalent, meningococcal C, and pneumococcal is planned. The measles, mumps, and rubella (MMR) vaccine is administered from 12 months, with a second dose between 16 and 18 months.

Which Vaccines Are Mandatory for Children Born After 2018?

Since January 1, 2018, French law has extended the number of mandatory vaccines for children born from this date. They have increased from 3 to 11 mandatory vaccines to be admitted to childcare facilities (crèche, school, daycare).

These 11 mandatory vaccines cover the following diseases: diphtheria, tetanus, polio, whooping cough, Haemophilus influenzae type b, hepatitis B, pneumococcal, meningococcal C, measles, mumps, rubella.

This measure was implemented to strengthen national vaccination coverage and protect children who cannot be vaccinated for medical reasons (contraindications). The concept of herd immunity is precisely based on this principle: the higher the vaccination rate in a population, the more limited the circulation of pathogens, thus protecting vulnerable individuals.

What Happens in Case of Non-Compliance?

A child not vaccinated according to the mandatory schedule cannot be admitted to childcare facilities. However, no criminal sanctions are currently planned for parents who do not vaccinate their child outside the context of collective care. This regulation aims more to encourage vaccination than to coerce it through fear.

Understanding Common Concerns About Vaccines

Many parents express concerns about the side effects of vaccines, the number of injections given at the same time, or the impact on the infant's immune system. These concerns deserve to be taken seriously and not dismissed out of hand.

Side Effects: What's Normal, What's Not

After vaccination, it is common to observe a slight fever, redness or swelling at the injection site, or temporary irritability. These reactions are generally benign and disappear within 24 to 48 hours. They indicate the immune system's reaction, which is precisely the desired goal.

However, certain signs should prompt a rapid consultation: very high fever, convulsions, an intense allergic reaction (generalized hives, breathing difficulties), or a significant alteration of the general condition. In these situations, contact your doctor or 15 (SAMU) immediately.

Are Too Many Vaccines Administered at the Same Time?

This is a question many parents ask themselves. Available scientific studies show that an infant's immune system is capable of responding to several antigens simultaneously without being overwhelmed. Grouping vaccines also reduces the number of medical visits and stress exposure for the child.
